Eyes
eyeball in normal health patient
Tried to measure the optic nerve sheathe diameter to the right, but the Butterfly's a bit limited for scans requiring really high resolution - >5 mm is relatively sensitive for increased intracranial pressures
Thyroid
clocked a nodule in the right lobe of the thyroid near the SCM in near field; refer to radiopaedia's nice summary of thyroid nodule classification; more often than not I wind up sending these folks for a formal unless it's very clearly low risk
hyperechoic nodule - when it's homogenous and hyperechoic + less than 1 cm in all dimensions it's generally low risk
thyroid nodule at about 1.5 cm; measured 0.4 by 0.7 cm --> I ordered a formal ultrasound and the radiologist recommended at least yearly surveillance
Lymph Nodes
cervical lymph node just superficial to jugular; normal lymph nodes are generally twice as long as they are wide with well defined borders and and echogenic hilum/hypoechoic cortex; sorta look like mini kidneys
mostly normal appearing LN, patient had diffuse LAD, though, and biopsy was positive for lymphoma
this is an atypical appearing lymph node that popped up just after a COVID-19 booster, went away after a few days
Just a large retromandibular node (seen at about 1 cm depth) associated with a sore throat - resolution's not great but you can see the characteristic hyperechoic center + hilum and the relatively hypoechoic cortex. Looks like a mini kidney.
Sinus
This is a normal air-filled sinus. There's a reverb artifact evident at about twice the depth of the maxillary periosteum - the sinus equivalent of A lines.
Again, normal sinus ultrasound with reverb artifact at twice the depth of the periosteum.
reverb artifact indicated aerated sinus
I use the MSK preset and make the depth 5 cm. Generally you'll see reverb at around 3-3.5 cm and if the cavity is fluid filled the back wall can be seen at about 5 cm.
these two sinus scans were in the same patient - the scan on the left demonstrates a reverb artifact indicating aeration (line at about 4 cm depth) while the scan on the right was positive for a fluid filled sinus demonstrating the back wall as a hyperechoic line at about 5 cm of depth
